IEP Worksheet

From the Care Parent Network comes this great worksheet for parents getting ready for their IEP meeting (in downloadable PDF format).

It gives you prompts for writing about your child and organizing your thoughts on what you would like to see on your child's IEP. A great resource for parents new to the IEP arena, and even for the "old timers" who might know what areas need discussed, but would like to have a form to gather the information in one place.
